MSRAL 2006 Convention Registration Information

1)  The quickest and easiest method to register is to use the SECURE shopping cart created through OUR paypal account.  You may use either your credit card OR your Pay  Pal account to pay.  Pay Pal is NOT required to use this service.  If you select this method, we will have you registered instantly and you will receive confirmation via email from Pay Pal as soon as you register. In addition, Cook receives a beautiful form with all the information we need to complete your ID badges for the conference.  Your personal information will not be used in any way except to confirm registration and provide critical conference information.  All information files will be removed after the conference.

2)  IF you do not have a pay pal account OR you do not wish to use your credit card, the second easiest method is to select your items from the shopping cart as though you were going to use a credit card, THEN select VIEW CART and print the results on your printer.  You may then MAIL the form to Cook Feldman at the address on the right of this page.  In addition, PRINT the REGISTRATION FORM and fill out the address and other information at the TOP of the page. 

The information we need from this form is your name, address, phone number, email address, and organization you are affiliated with.
